A fast and easy system for completion of the CVI form which instigates registration for visual loss.

The system has been developed by Moorfields Eye Hospital and is their own electronic solution to the CVI form which is produced by the Department of Health which is used for certification of a person as Sight Impaired or Severely Sight Impaired.

In order to use the ECVI system within Moorfields Network or remotely, users must be setup and given access by our administration team. If you have already been given access then click the relevant button to be redirected to the certification page.

For those who have not been given access and wish to take part in the pilot system, you will need to make sure that you have the following requirements:

For Access via Moorfields Network

  • Access to the MEH network including the Clinical Services Page
  • A valid MEH User Name

For Remote Access

  • A valid User Name and Password provided by our administration team

Please ensure that your hospital is sending copies of the Certificate of Vision Impairment (CVI) for epidemiological analysis to the correct address:

  • CVIs provide data for public health preventable sight loss indicator
  • The Certifications Office used to be based at Empire House, 133-144 City Road, London EC1V 2QN. In August 2007 office has moved to main building of the Moorfields Eye Hospital. The CVI form (CVI 2007) was updated with new address, however several eye clinics and ophthalmologists use the older versions of the CVI (2003, 2005) which has the incorrect address.
  • Since 2007 the Royal Mail has provided a safeguard automatic redirection which ensures that confidential information does not reach non-NHS premises. Automatic redirection of mail from old address runs out on 5th March 2014 and there is no scope for further extension.
